What you share

Forms collect your name, email, optional phone number, and the information you choose to provide about your location, nomination, offer to help, or question. Initial nominations ask for the veteran’s first name and last initial, general location, and a description of the need. Please do not submit Social Security numbers, medical records, military documents, bank information, or exact home addresses.

How information is used

The Decks4Vets team uses submissions to review nominations, evaluate offers to help, respond to questions, and administer the launch list. People who join the launch list consent to Decks4Vets email updates. Supporters can also indicate interest in future monthly giving without committing to a payment. When a campaign link is used, the form can record the campaign name and general source (for example, email or Facebook) so we can see which outreach is useful. We do not sell form information. We do not publish a veteran’s identity or story from a nomination; separate permission will be requested before any public story or photograph is shared.

Storage and access

Submissions are stored in the website’s database and are available to authorized site operators. Hosting providers process the information needed to run and secure the site. The form service keeps a daily keyed hash of the submitting connection’s IP address to limit spam; the raw IP address is not stored in the submission record. Hosting infrastructure may separately maintain operational logs. Only a confirmation reference, not the contents of a submission, is returned after a successful save.

Retention and your choices

We intend to review inactive inquiries after 12 months and remove information that is no longer needed, subject to any legal recordkeeping requirements. To request correction, deletion, or removal from launch updates, use the contact form and select “Privacy / remove my information.” Include the email used on the original submission. We may verify your request before changing records.

Payment and other websites

Donation checkout is not open. This site does not collect card or bank details. If giving opens through an external payment provider, that provider’s terms and privacy notice will apply. Links to other websites have their own policies.

Children

These forms are intended for adults. If you are under 18, ask an adult to submit a nomination or inquiry on your behalf.
